Elective Posting (Day1)

16-March.Dunno the weather cos i work from 8am-5pm non-stop. phewww....

THe day start off with a breakfast at 7am (wow,so early...). THen, i reached the hospital at 730am. With the help of the security guard, i safely arrive at the physiotherapy unit. 8am, I introduce myself to the head of unit. I was asked to see the Hospital Director first. Outside the Director's office, I saw one guy and four girls that looked as blur as me walking into his office. Hmmm...i approach them and find out that they are medical student from UMS and they are having their elective posting too. same date and duration somemore.haha.

Wow, the physiotherapy unit in cbu wass so so so efficient. I thought i would be like my other colleagues doing very little thing in my elective posting, who knows??? I was given a schedule on what im goin to do everyday and the person in-charge. I need to sign in 8am and sign out 5pm. OMG...

First day, i was asked to follow a physiotherapist on the outpatient unit for stroke patient. the first patient have stroke secondary to pontine haemorrhage. I think he has nystagmus.wow,good finding...then, i help to apply hot pad on a patient with osteoarthritis. I palpated and feel the crepitation on moving her knee joint,she has "knot knee".

In the afternoon, PPD stuffs come in. A patient come in with paraplegia up to T7-T8 level. What is the cause??? It's Tuberculosis (TB). She speak Bahasa Iban. THough Im sarawakian, i was brought up in a chinese school. I really cannot understand bahasa Iban though it sounds like Bahasa Malaysia, but could hardly understand. Communication Barrier...so, if wanna practise in sarawak next time i shall start learning bahasa iban liao.

THis patient come from rural area where she need2 take express (means ship) to come to hospital. THerefore, it's impossible for her to come evryweek. THe physiotherapist is very considerate, he taught the patient for more than 1.5 hours, so that she can come fortnightly.

My mine was bombarded with questions. Why she got tuberculosis??? Why both her legs paralysed?? Why is she on CBD??? WHY????

The answer is "lackof doctors in rural area" . She was not immunised (BCG vaccination programme fail in rural area). She was not treated appropriately. ( one full course of anti-TB drugs was so easily available in the city, but in rural area....haiz...).

Events coming up would be : Backcare programme, homevisits, visit to NGO (Agape centre), Inpatient physiotherapy (ICU, Surgery, Orthopedic, medicine...)
